TMDX's PAD Aviation investment expands its OCS model into Europe, aiming to boost organ logistics, utilization and adoption.
Strata Critical Medical is rated a buy, driven by expanding market share, operational efficiencies, and a comprehensive suite of transplant logistics and clinical services. SRTA's asset-light-to-moderate model, strategic acquisitions, and focus on NRP technology position it to benefit from industry consolidation and secular tailwinds. Gross margin improvement is a key near-term catalyst, with management incentivized to reach 25% margins by 2027; Q1 2026 gross margin reached 21%.
TMDX is advancing its OCS platform and transplant ecosystem, but margin pressure from expansion investments remains a key concern.

TransMedics Group, Inc. is a pioneering commercial-stage medical technology company deeply invested in revolutionizing the therapy for patients suffering from end-stage organ failure across the globe, including in the United States. Founded in 1998, with its headquarters located in Andover, Massachusetts, TransMedics is dedicated to advancing the field of organ transplantation through its innovative Organ Care System (OCS). This cutting-edge system offers a dynamic approach to organ preservation, optimization, and monitoring by mimicking near-physiologic conditions outside the human body using specialized, proprietary technology. By pushing the boundaries of medical technology, TransMedics Group, Inc. seeks to improve the outcomes of organ transplants and broaden the range of viable organs available for transplantation, thus saving more lives and improving the quality of life for patients with life-threatening organ failures.
This product is designed for the preservation of standard criteria donor lungs intended for double-lung transplants. By maintaining the donor lungs in a near-physiologic state, the OCS LUNG system increases the potential pool of viable organs for transplantation, thereby addressing the critical shortage of transplantable lungs.
OCS Heart is an advanced technology geared for the preservation of heartbeating donor (DBD) hearts that may be considered unsuitable because of the limitations associated with cold storage. The system additionally offers a unique capability for the ex vivo reanimation, functional monitoring, and beating-heart preservation of hearts from donors after circulatory death (DCD), effectively expanding the heart donor pool by utilizing organs that may not have been considered for transplantation previously.
Geared towards the preservation of donor livers, including those from DBD and DCD donors, the OCS Liver system operates by recreating near-physiological conditions that sustain the liver outside of the human body. This technology is pivotal in increasing the usability of donated livers for transplantation, potentially reducing the waiting lists for liver transplants significantly.
TransMedics has also established a national OCS program that acts as a comprehensive solution for organ retrieval. This includes outsourced services that cover the entire process of organ procurement, extending from aviation and ground transportation to various coordination activities, thus streamlining the organ transplantation logistics chain.
In addition to the innovative OCS technology, TransMedics provides a suite of organ management and logistics services. These services are designed to facilitate the complex process of organ transportation and management, ensuring that organs arrive at their destination in optimal condition ready for transplantation. This comprehensive service package includes aviation and ground transportation, among other critical coordination activities necessary for successful organ transplantation.
